Geriatric Medicine, Sunnyvale (PAFMG2017-252)
Palo Alto Foundation Medical Group (PAFMG) has a full time opportunity for a BE/BC fellowship-trained Geriatrician (IM or FP) to join our growing Geriatric Medicine Department in Sunnyvale, California. Applicants should have a current California Medical License or able to obtain a California Medical License, DEA Certificate and eligibility to participate in Medical/Medicare. This physician will work closely with our interdisciplinary team such as LVNs, MAs, skilled nursing facility coordinators, and office manager to provide exceptional patient care.
Our newer colleagues will work alongside us and provide post-acute and long-term patient care at our partnering skilled nursing facilities. Physician will also have an opportunity to provide care in the Geriatrics consultation clinic in our Sunnyvale campus.
Our group is passionate about supporting patients throughout the continuum of care. In addition, we are all very involved in quality improvement and research projects that have both local and national impact and this physician will have an opportunity to be involved in these projects.
